Intent Form
The intent form may be used as is OR you may wish to personalize the form by adding your congregation's information in the empty space on the left side. Congregations are to collect the intent cards, then share with the synod the total amount and number of giving units. Contributions are to be collected by the congregations and ultimately provided in one lump sum to the synod office.
